TUDCA is a taurine-conjugated bile acid & chemical chaperone that inhibits ER stress, supports mitochondrial integrity, & exerts hepatoprotective effects; human RCTs demonstrate liver enzyme normalization in cirrhosis & improvements in hepatic insulin sensitivity.

The evidence for TUDCA includes small RCTs showing benefits for liver enzymes and insulin sensitivity. However, the overall quality of studies is low, and more research is needed to confirm these effects.

A double-blind RCT (n=18, 750 mg/day, 6 months) found TUDCA significantly reduced serum ALT, AST, & ALP versus baseline, outperforming UDCA on biochemical normalization; both groups showed increased albumin. TUDCA was concluded to be safe & more effective than UDCA for biochemical improvement.

A placebo-controlled RCT (n=20, 1750 mg/day, 4 weeks) found TUDCA improved both hepatic & muscle insulin sensitivity by approximately 30% versus placebo; adipose tissue insulin sensitivity was unchanged; ER stress markers did not change, suggesting an alternative mechanism beyond classical chaperone activity.
