Tirzepatide
Tirzepatide is a synthetic 39-amino acid dual agonist at both the GLP-1 and GIP receptors, engineered as a single peptide with a C20 fatty diacid attached to the lysine at position 20 to extend half-life for once-weekly dosing. The GIP receptor agonism adds complementary pathways for insulin secretion, glucagon suppression, and adipose tissue lipolysis to the GLP-1 effects on gastric emptying and satiety, producing a synergistic weight loss profile. The SURMOUNT-1 trial in adults without diabetes showed mean body weight reductions of 20.9% at the 15 mg dose over 72 weeks — surpassing semaglutide in head-to-head pharmacodynamic comparisons. It is FDA-approved as Mounjaro for type 2 diabetes and as Zepbound for chronic weight management; GI adverse effects mirror the GLP-1 class with nausea being the primary tolerability limitation.
