Tirzepatide

Tirzepatide is a synthetic 39-amino acid dual agonist at both the GLP-1 and GIP receptors, engineered as a single peptide with a C20 fatty diacid attached to the lysine at position 20 to extend half-life for once-weekly dosing. The GIP receptor agonism adds complementary pathways for insulin secretion, glucagon suppression, and adipose tissue lipolysis to the GLP-1 effects on gastric emptying and satiety, producing a synergistic weight loss profile. The SURMOUNT-1 trial in adults without diabetes showed mean body weight reductions of 20.9% at the 15 mg dose over 72 weeks — surpassing semaglutide in head-to-head pharmacodynamic comparisons. It is FDA-approved as Mounjaro for type 2 diabetes and as Zepbound for chronic weight management; GI adverse effects mirror the GLP-1 class with nausea being the primary tolerability limitation.

Evidence is from research or clinical settings — does not imply safety outside supervised contexts.

Tirzepatide has strong evidence supporting its effectiveness in glycemic control, weight reduction, and cardiovascular risk reduction. While it shows promise for liver health and obstructive sleep apnea, further research is needed to fully understand its long-term effects and safety profile.
