TUDCA is a taurine-conjugated bile acid and chemical chaperone that inhibits ER stress, supports mitochondrial integrity, and exerts hepatoprotective effects; human RCTs demonstrate liver enzyme normalization in cirrhosis and improvements in hepatic insulin sensitivity.

A double-blind RCT (n=18, 750 mg/day, 6 months) found TUDCA significantly reduced serum ALT, AST, and ALP versus baseline, outperforming UDCA on biochemical normalization; both groups showed increased albumin. TUDCA was concluded to be safe and more effective than UDCA for biochemical improvement.
A placebo-controlled RCT (n=20, 1750 mg/day, 4 weeks) found TUDCA improved both hepatic and muscle insulin sensitivity by approximately 30% versus placebo; adipose tissue insulin sensitivity was unchanged; ER stress markers did not change, suggesting an alternative mechanism beyond classical chaperone activity.
